1. Warning, Cautions and Precautions

WARNINGS AND PRECAUTIONS:

• The ArchForm Orthodontic Software System is intended to be used by an orthodontist or qualified dental health professional.
• The aligners must be placed by an orthodontist or qualified dental health professional to insure proper fit.
• Improperly fitted aligners or orthodontic appliances could result in excessive pressure on the dentition and oral structures resulting in injury.

ADVERSE EVENTS:

While the use of this software by itself has no known adverse events for the user. This software is often used in conjunction with Dental Aligners and other related dental products. Users should be trained in orthodontic or dental health practices and have knowledge of applicable adverse events related to orthodontic treatment prior to the use of this software.

CONTRAINDICATIONS:

There are no known contraindications to this device.
